Court Forms Required For Maryland MD Family Law Cases


Domestic Case Information Report (DCIR)

If this DCIR is not filed with the Complaint, the Complaint may be rejected by the clerk and the Complaint may need to be re-filed, even if this is for an Uncontested case that has a signed Agreement



Financial Statement Form (Short-Form)

--Required for all Contested Divorce and Child Support Cases

--Must be filed with the pleading requesting support and must be updated before Trial



Financial Statement Form (Long-Form)

--Required for all contested Child Support cases where the parties have a combined monthly income of at least $30,000 a month, and in any case where Alimony has been requested

--Must be filed with the pleading requesting support and must be updated before Trial



Joint Statement Of The Parties Concerning Marital And Non-Marital Property
(for Contested Divorce cases only)

--Required for all Contested Divorce cases

--Must be filed at last 10 days before the Final "Merits" Trial, unless the Scheduling Order in the case states another time, or another time is fixed by the Court

--Some Courts issue Scheduling Orders that require this form to be filed much ealier than the Trial, such as at least 10 days before a Settlement Conference, or another time is fixed by the Court.



Joint Statement Of The Parties Concerning Decision-Making Authority And Parenting Time
(for Contested Custody cases only)

--Required for all Contested Custody cases

--Must be filed at last 10 days before the Final "Merits" Trial, unless the Scheduling Order in the case states it must be filed at an ealier time, such as at least 10 days before a pendente lite hearing in the case, or, if there is no pendente lite hearing, then possibly at least 10 days before a Settlement Conference, or another time is fixed by the Court



Notice Of Restricted Information Form

This form must be filed with the Financial Statement Form and the two Joint Statements stated above, or the clerk will likely Reject these filings.
